What is the Community Supports Waiver?
This state program provides participants with a wide range of services designed to increase independence and quality of life. It allows participants to hire personal care assistants, receive professional support, and use services tailored to their individual needs.
After submitting your application and being approved for participation, you will receive a letter from your regional office. At a meeting with your Community Care Services (CCS) coordinator, you will discuss and select the types of assistance that best suit your goals and lifestyle.
The main goal of the program is to help people with special needs live the life they choose.
Who does the program support?

How do I apply?
